Here’s what to include:
Basic Information
- Pet’s Name (and nickname they respond to)
- Species/Breed
- Age & Weight
- Gender (spayed/neutered?)
- Microchip Number
If your pet is chipped, note the number and registration provider.
Veterinary Info
- Primary Vet Name & Clinic
- Clinic Phone Number & Address
- After-Hours/Emergency Vet Info
Include your preferred emergency hospital if your regular vet is closed.
Medical History
- Current Medications
Include names, dosages, and schedules. - Medical Conditions
Allergies, past surgeries, or ongoing treatments. - Vaccination Status
Especially rabies, bordetella, and DHPP for dogs.
Behavioral Notes
- Fears or Triggers (e.g., thunder, vacuum, strangers)
- Aggression/Anxiety
Include if they’re leash reactive, food aggressive, etc. - Escape Habits
Let us know if they’re an expert fence-jumper or door-dasher!
Contact Info
- Primary Owner Name & Number
- Backup Emergency Contact
Someone local who can make decisions if you’re unreachable.
Care Instructions
- Feeding Schedule & Amount
- Walking/Potty Routine
- Favorite Treats or Toys
This helps us keep them calm and happy while you’re away.
